Pulled Hamstring: Why the First Week Decides How Fast You Get Back

What a Hamstring Strain Actually Is

You are chasing down a ball at Nelson Park, or pushing the last interval of a track session, and something at the back of your thigh grabs. Sharp, sudden, unmistakable. You pull up, put a hand on the back of your leg, and you already know. A pulled hamstring is one of the most common sports injuries we see, and effective hamstring strain treatment in Burlington comes down to one thing most people get wrong: what you do in the first week matters more than how long you rest.

A hamstring strain is a tear in the muscle fibres at the back of the thigh. It happens fast, usually during sprinting, kicking, or a sudden lunge for a ball, when the hamstring is lengthening and contracting at the same time. That is the moment of highest tension, and it is where the tissue gives way. This is a different injury from the deep, nagging sit bone pain that creeps in over weeks. That one is proximal hamstring tendinopathy, and it needs a different plan entirely. A strain is sudden. A tendinopathy builds. Getting that distinction right on day one changes everything about how we treat it.


What Causes a Pulled Hamstring

Hamstring strains are not random bad luck. They cluster around a few predictable situations:

  • Sprinting at high speed - The late swing phase of a sprint, just before your foot hits the ground, is where most hamstring strains happen.

  • Doing too little, not just too much - This surprises people. Athletes who have not been exposed to regular high-speed running are often the ones who tear. The tissue never built the tolerance.

  • Returning too early from a previous strain - The single biggest risk factor for a hamstring strain is having had one before.

  • Fatigue late in a game - Tired muscles absorb force poorly.

  • A cold start - Going straight into a sprint from the bench with no build-up.

That second point deserves a moment. Research following professional footballers across three seasons found that lower accumulation of high-speed running distance was associated with hamstring strain injury. The instinct to simply back off and do less can quietly leave the tissue less able to handle the one moment it really has to. This is why we do not just rest people in our Burlington physiotherapy clinic. We rebuild capacity.


Signs and Symptoms of a Hamstring Strain

Most people know the instant it happens. Still, the details matter for grading the injury.

What You Will Notice Right Away

  • A sudden sharp pain or a pulling sensation at the back of the thigh.

  • Sometimes an audible or felt pop.

  • Having to stop immediately. You cannot run through it.

  • Pain when you stretch the leg out straight or contract the hamstring.

  • Bruising appearing over the following days, often tracking down toward the knee.

The One Sign That Changes the Timeline

Where the pain sits matters enormously. The closer the point of peak tenderness is to the ischial tuberosity, the sit bone at the top of the hamstring, the longer the recovery tends to take. This is one of the most useful things a physiotherapist can tell you in the first session, and it does not require an MRI. It requires careful hands and knowing what to feel for.

Red flags worth an urgent assessment: a large defect you can feel in the muscle, severe bruising high in the buttock, an inability to bear weight, or a sensation that the leg gave way completely. A full tendon avulsion is uncommon, but it is managed very differently.


What the Research Says About Recovering Faster

A randomised controlled trial by Askling and colleagues followed 56 elite sprinters and jumpers with MRI-confirmed hamstring injuries. Athletes were randomly assigned to one of two rehabilitation protocols, both started 5 days after injury. The group given the lengthening-based, eccentrically biased protocol returned to full training in a mean of 49 days. The group given the conventional protocol took a mean of 86 days. That is a difference of more than five weeks, from the same injury, on the same timeline, differing only in how the muscle was loaded. Only two re-injuries occurred across the whole study, both in the conventional group. Askling C, Tengvar M, Tarassova O, Thorstensson A (2014). The takeaways that actually matter for someone in Burlington nursing a pulled hamstring:

  • Loading the muscle at length, early, was the difference. Not rest.

The lesson is not that you should aggressively stretch a fresh tear. It is that an early, controlled eccentric stimulus puts the tissue in a state far better prepared for the loads waiting for it on the field.

Early exercises are performed slowly, in controlled range, with no pain provocation. The muscle is asked to work at length, not simply shortened. Load and speed progress from there.

Rebuilding Capacity for Speed

Strength alone does not protect a hamstring. Sprinting does. Late-stage rehab reintroduces high-speed running progressively, because that is the exact demand that broke the tissue in the first place, and the one it has to tolerate again. This is a core part of sports injury rehabilitation, and it is the stage most commonly skipped.

  • Isometric holds early, progressing to slow eccentric work at length.

  • Hip-dominant and knee-dominant loading, since they build the muscle differently.

  • Trunk and pelvic control, because the hamstring does not work alone.

  • Graded return to running, then striding, then true sprinting.

  • A clear, objective clearance test before return to sport.

Hands-on work has a supporting role. Neither replaces the exercise. They make the exercise easier to do well.


Why So Many Hamstring Strains Come Back

Re-injury rates for hamstring strains are notoriously high, and the reason is almost always the same. People return when the pain stops. Pain stops long before capacity returns.

A hamstring that feels fine walking around Millcroft can still be nowhere near ready for a full-speed sprint after a loose ball. The gap between comfortable and capable is where re-injuries live. This is why we use objective criteria rather than how the leg feels on the day, and why we do not clear anyone who has not sprinted, in rehab, under supervision, before they sprint in a game.


What to Expect at Your First Appointment

You will spend a full 60 minutes one-on-one with Javier. No assistants, no rotating between three patients at once. We take a detailed history of exactly how the injury happened, because the mechanism tells us a great deal. Then a thorough physical exam: palpation and mapping, strength testing through range, neural screening, and a lumbar and pelvic assessment.

You leave with a working diagnosis, a realistic timeline, and the first phase of your program in hand. If we think imaging or a medical referral is warranted, we will tell you that too.


Five Things to Do in the First Week

  1. Offload early, but briefly. The first 2 to 3 days are for protecting the tissue. Use crutches if walking is genuinely painful. This phase is short, not a lifestyle.

  2. Do not stretch it hard. Aggressively pulling on a fresh tear does not help it heal and can aggravate the injury. Gentle, pain-free movement is a different thing entirely.

  3. Start moving within a few days. Pain-free range of motion and gentle isometric contractions can usually begin early. Sitting still for three weeks is how a 7-week recovery becomes a 12-week one.

  4. Note exactly where it hurts. Pain very high, up near the sit bone, generally means a longer road. Useful information for your physiotherapist.

  5. Get it assessed properly before you plan a return date. Grading the strain accurately at the start is what makes the timeline realistic.

When to See a Physiotherapist for a Hamstring Strain

Soon. Ideally within the first few days. The Askling trial started its rehabilitation at 5 days post-injury, and that early window is exactly when the right loading decisions pay off most. Waiting three weeks to see someone, hoping it settles on its own, means arriving at the clinic having already lost the most valuable phase of the recovery.
