That Knot Between Your Shoulder Blades? Mid Back Pain Treatment in Burlington
What Mid Back Pain Actually Is
It usually lands right between the shoulder blades. A deep ache that builds through the workday, a knot you keep trying to dig out with your thumb or the corner of a doorframe, or a sharp catch when you twist to grab something off the back seat. If you are looking for mid back pain treatment in Burlington, you are dealing with something far more common than most people realize, and something that gets a lot less attention than neck or low back pain. At Pursuit Physiotherapy in Burlington, we see it every week, very often in people who sit for a living.
Your mid back, or thoracic spine, is the section between the base of your neck and the bottom of your ribcage. It has 12 vertebrae, and each one connects to a pair of ribs. That design makes it the most stable part of your spine, which is great for protecting your heart and lungs, but it also means there are a lot of small joints that can get stiff and irritated. Most mid back pain is mechanical: it comes from those joints and the muscles around them, and it changes with movement, position, and breathing.
It is also common. A systematic review of 33 studies found that up to about a third of people deal with thoracic spine pain over the course of a year, depending on how it is defined. Yet neck pain and low back pain each have detailed clinical guidelines telling clinicians what works, and the mid back does not. That gap is part of why mid back pain so often gets brushed off as "just posture" or treated with a massage and nothing else. It deserves a proper plan.

Why the Mid Back Gets Sore
Thoracic pain rarely has one dramatic cause. More often it is a load the mid back was not ready for, stacked on top of a back that has not been asked to move much in a while. Common contributors include:
Long hours in one position. Desk work, driving, and gaming all keep the mid back still for hours at a time. No single posture is the villain. Staying in any posture too long is.
Irritated rib joints. Each rib meets the spine at two small joints. When one of them gets cranky, you feel a sharp, localized pain that can wrap around the side of your chest and catch on a deep breath, a cough, or a twist.
Sudden strain. A heavy lift, an awkward reach, a golf swing, a hockey hit, or the first big snow shovelling session of the winter.
Pain referred from the neck. Joints in the lower neck can send pain into the area between the shoulder blades, and a pinched nerve in the neck often shows up as pain near the shoulder blade before it ever reaches the arm.
Stress and poor sleep. Research links thoracic pain with psychological and lifestyle factors as well as physical ones. Tension tends to settle across the upper and mid back.
Deconditioning. Muscles around the shoulder blades and spine that rarely get challenged have less capacity when demand goes up.
With winter on the way, that third point is worth flagging. Shovelling is a classic mid back trigger: bending, twisting, and throwing, usually done cold and in a hurry.
Signs and Symptoms of Mid Back Pain
A dull ache or burning between the shoulder blades that builds through the day
Stiffness when you twist, reach overhead, or turn to look behind you
A sharp, pinpoint pain beside the spine that catches on a deep breath, sneeze, or cough
Pain that wraps around the side of the ribcage
Tender spots or "knots" in the muscles beside the spine
Relief when you move around, and a flare when you sit still again
When Mid Back Pain Needs a Doctor First
The mid back sits right beside your heart, lungs, and several abdominal organs, so it is the one area of the spine where we are especially careful to rule out problems that are not musculoskeletal. If you have chest pain or pressure, pain that comes on with exertion, shortness of breath, sweating, or nausea, call 911. See your doctor promptly if your pain comes with a fever, unexplained weight loss, a history of cancer, a burning band of pain with a rash, pain that does not change at all with movement or position, a fall or injury (especially if you have osteoporosis), or new numbness, weakness, or balance problems in your legs. Screening for these is built into every assessment we do.
What the Research Says About Treating Mid Back Pain
Because the mid back has been studied far less than the neck and low back, good trials are worth paying attention to. A 2023 randomized controlled trial asked a simple question: does adding spinal manipulation to an exercise program help people with thoracic pain more than exercise alone?
One hundred adults with mid back pain, a stiff thoracic spine, and pain when the spine was compressed were split into two groups. Both groups did the same four thoracic strengthening exercises with a resistance band and received advice on their work setup, twice a week for four weeks. One group also received a thoracic manipulation. The manipulation group improved faster: after eight sessions their pain had dropped from about 6.5 out of 10 to about 2.3, compared with 3.4 in the exercise-only group. By the 12-week follow-up, the difference between the groups was gone.
Waqas MS, Karimi H, Ahmad A, Rafiq S, Anwar N, Liaqat S (2023). The effects of spinal manipulation added to exercise on pain and quality of life in patients with thoracic spinal pain: a randomized controlled trial. BioMed Research International, 2023, 7537335. Read the full study.
What this means in practice:
Exercise did the heavy lifting. The exercise-only group roughly halved their pain by the end of treatment, and they kept improving after the sessions ended.
Hands-on treatment speeds things up. Pain in the manipulation group dropped noticeably after the very first session, which makes it a lot easier to move and get the exercises done.
Manual therapy works best as part of the plan, not the whole plan. Its extra benefit had faded by 12 weeks. That lines up with best-practice guidelines for musculoskeletal pain, which put exercise and education first and use hands-on care to support them.
That is exactly how we approach mid back pain at Pursuit: hands-on work to calm things down and get you moving, and an exercise plan to make sure it stays that way.
Mid Back Pain Treatment in Burlington: How Physiotherapy Helps
Our approach follows that evidence, adapted to what we find in your assessment rather than a set protocol.
Finding the Real Source
First we make sure it is actually coming from the spine. Then we narrow it down: testing the thoracic joints level by level, checking the rib joints and how your ribcage moves when you breathe, screening the neck for referred pain, and watching how your shoulder blades move. If your symptoms seem to start higher up, our guide to treating neck pain covers that side of things in more detail. We also ask about your work, sleep, stress, and training, because they shape the plan as much as any stiff joint does.
Hands-On Treatment That Gets Things Moving
When testing points to stiff, irritable joints, manual therapy in Burlington is usually where we start: mobilization or manipulation of the thoracic spine and ribs, plus soft tissue work through the muscles guarding the area. If the idea of a thrust technique makes you nervous, that is completely fine. Gentler mobilizations are a good option, and we choose techniques with you, not for you.
For stubborn muscular pain, medical acupuncture can be a useful addition. Needling around the ribcage demands real precision, and Javier teaches medical acupuncture to other health professionals at McMaster University, so it is used carefully and only where it earns its place.
Exercise That Makes the Change Stick
This is the part that keeps mid back pain from coming back. Your physio-designed exercise program will usually include thoracic rotation and extension to restore movement, plus strengthening for the muscles that pull your shoulder blades back and hold them steady, the same targets used in the trial above. It starts where you are and progresses as you do, whether your goal is a pain-free workday, lifting without guarding, or a round of golf you don't pay for the next morning.
What to Expect at Your First Appointment
Your first appointment at Pursuit is a full hour, one-on-one with Javier Diaz, registered physiotherapist. No assistants and no double booking. That hour covers a thorough history, screening for anything that needs a doctor instead, a full physical assessment of your spine, ribs, neck, and shoulders, an honest explanation of what is driving your pain, and the start of treatment the same day. You leave with a clear plan and exercises you can start that evening.
Five Ways to Ease Mid Back Pain Today
Break up your sitting. Set a timer for every 30 to 45 minutes. Stand up, reach both arms overhead, and twist gently each way. Frequent movement beats chasing a perfect posture.
Try an open book stretch. Lie on your side with your knees bent and both arms out in front of you. Slowly open your top arm across your body toward the floor behind you, following your hand with your eyes, then bring it back. Ten slow reps each side, within comfort.
Lean back over your chair. Sit in a chair whose backrest stops around your mid back, support your head with your hands, and gently arch back over the top of the backrest. Hold for a breath, return, and repeat 8 to 10 times.
Breathe into your ribs. Place your hands on the sides of your ribcage and take five slow breaths, feeling your ribs widen into your hands. It is a gentle way to move stiff rib joints, and a warm shower or heat pack first can make it easier.
Start loading it. Band pull-aparts or rows, two or three sets of ten, a few times a week. And when the snow comes, push rather than throw, keep the load close to your body, and turn your feet instead of twisting your back.

When to See a Physiotherapist for Mid Back Pain
If your mid back pain has lasted more than a week or two, keeps coming back every few months, catches every time you breathe deeply (once your doctor has ruled out other causes), or is starting to limit your work, training, or sleep, it is time to have it properly assessed. Mid back pain tends to respond well to the right plan, and getting on top of it early is a lot easier than untangling it after months of guarding. We see patients from Burlington, Millcroft, Aldershot, Waterdown, Oakville, Milton, and Hamilton.
