What Is a Calf Strain?

You push off to chase a ball, spring up the stairs, or pick up the pace on a run, and something grabs in the back of your lower leg. Some people describe it as a sudden cramp that will not let go. Others are convinced someone kicked them from behind. Either way, a calf strain, also called a pulled calf muscle, is one of the most common injuries we treat at Pursuit Physiotherapy in Burlington, especially through the summer when running, tennis, pickleball, and soccer are in full swing.

Your calf is actually a group of muscles. The two that matter most are the gastrocnemius, the larger visible muscle, and the soleus, a deeper and flatter muscle underneath it. Both feed into the Achilles tendon, and together they act like a spring every time you walk, run, or jump. A strain happens when the muscle is loaded beyond what it can currently handle and some of its fibres tear.

Here is the part most people underestimate: the first calf strain is rarely the real problem. The real problem is the second one, and the third. Calf strains are notorious for coming back, and each repeat tends to cost more time than the one before it. Good treatment is not just about settling the pain this week. It is about making sure this is the last calf strain you deal with.


Why Calf Strains Happen

A calf strain is almost never bad luck. There is usually a mismatch between what the muscle could handle and what was asked of it. Common culprits include:

  • A sudden jump in activity - ramping up running volume quickly, adding hills or speed work, or going from the couch to a full pickleball tournament weekend.

  • Explosive push-off movements - sprinting, lunging for a shot, or accelerating from a standstill. The calf handles enormous forces during push-off, which is why racquet sports and soccer produce so many of these injuries.

  • Fatigue - strains cluster late in games and long runs, when the muscle is tired and its capacity drops.

  • Age - calf strains become noticeably more common from the mid 30s onward, and research shows older athletes are more likely to re-injure.

  • Previous injury - a prior calf strain, or even a recent ankle injury on the same side, leaves the area more vulnerable if rehab was cut short.

What the Research Says About Calf Strain Recurrence

A landmark study followed 149 calf muscle strains in elite Australian Football players over four consecutive seasons, tracking clinical findings, MRI results, recovery timelines, and re-injuries.

The soleus was involved in 72% of calf strains. Players with a previous calf strain were nearly 7 times more likely to suffer a recurrence within two seasons, and 63% of recurrences happened at the exact same spot as the original injury. Green et al., American Journal of Sports Medicine, 2020.

A few takeaways worth pulling out of that study:

  • Injury history was the strongest predictor of the next injury. Older age also increased recurrence risk.

  • Strains that started during running, and strains involving the deeper connective tissue of the muscle, took meaningfully longer to recover, in some cases adding up to 11 extra days.

  • Returning before the muscle was truly ready was a recipe for prolonged and incomplete recovery.

In plain terms: the calf keeps score. If a strain is rushed or under-rehabbed, the odds of doing it again are heavily stacked against you. That matches what we see clinically, and it is why our focus is on rebuilding real capacity, not just waiting for pain to fade.

This is the step most people skip on their own, and it is exactly where recurrences are born.

5 Ways to Manage a Calf Strain at Home

  1. Keep moving within comfort. Complete rest beyond the first couple of days works against you. Walk as normally as pain allows, and use a heel lift in your shoe temporarily if it eases push-off pain.

  2. Start gentle loading early. Pain-free calf raises with both legs, progressing gradually toward single-leg raises, are a reasonable early step for mild strains.

  3. Resist the urge to stretch aggressively. Freshly injured muscle fibres do not respond well to hard stretching. Restore length gradually as the pain settles.

  4. Train both knee positions. Bent-knee calf raises bias the soleus and straight-knee raises bias the gastrocnemius. Most do-it-yourself rehab misses the soleus entirely, which is the muscle injured most often.

  5. Do not sprint on it to "test it." The muscle needs to earn its way back through strength and hopping before running, and through running before sprinting.


When to See a Physiotherapist

If your calf pain is severe, you cannot rise onto your toes, the injury keeps re-flaring every time you return to activity, or this is not your first calf strain, get it assessed. The research is blunt about what happens when these injuries are under-rehabbed, and a repeat strain usually costs far more time than doing it right once.
